The MyHealth Passport website is no longer available

Patients and caregivers at The Hospital for Sick Children (SickKids) seen after June 2018 can sign up for a MyChart account to access parts of their or their child’s electronic health record or can inquire with the Health Information Management department to request access to information from their or their child's patient health record.
